Chapter 3 - The Surgeon's Gambit

Back at the private hospital, high above the bustling streets of the trauma center, Dr. Ivan Sterling stood silently beside Jessi Vance’s hospital bed in the intensive care unit.
Jessi was slowly regaining consciousness. Her color had returned from the ghastly bluish-gray of near-death to a pale, natural pink. Her eyelids fluttered, and she let out a soft, raspy groan as she fought her way back through the heavy fog of sedatives and emergency antidotes.
Ivan reached out, gently taking her frail, cold hand in his. "Easy, Jessi. Take it slow. You're safe now. You're in the hospital."
Jessi’s eyes slowly cracked open, staring up at the acoustic ceiling tiles before shifting to focus on Ivan’s tired, lined face. A tear slipped down her cheek.
"Ivan...?" she whispered, her voice barely audible above the rhythmic hiss of the oxygen monitor. "What... what happened to me? I couldn't breathe... it felt like my chest was on fire."
"You went into severe anaphylactic shock," Ivan said gently, stroking her hand. "Someone applied a Dexopraline transdermal patch directly to your chest while you were sleeping at home."
Jessi’s eyes widened in sheer horror. She instinctively tried to lift her hand to touch her collarbone, remembering the absolute terror of the allergy that had haunted her entire adult life. "Dexopraline...? But... but I never touch that drug. I avoid it like the plague. You know that, Ivan. We almost died together remembering that allergy decades ago."
"I know, Jessi," Ivan said, his jaw tightening as cold fury burned in his chest. "Which means someone put it there on purpose."
Jessi stared at him, the horrifying implication of his words settling over her mind like a heavy shroud. Her expression shifted from confusion to a deep, profound heartbreak that hurt more than the physical trauma. "Luca...?" she breathed out, her voice breaking into a sob. "Did Luca... did my husband do this to me?"
Before Ivan could answer, the heavy wooden door of the ICU room swung open with a sharp click.
Luca Vance stepped into the room. He had changed his coat, but his hair was slightly disheveled, and his face wore the polished, rehearsed mask of a deeply concerned husband rushing to the bedside of his beloved wife.
"Jessi! Oh, thank God," Luca cried out, rushing toward the bed with open arms, his eyes wet with artificial tears. "I’ve been beside myself all morning! The doctors wouldn't let me in, they wouldn't tell me anything—are you alright, my love? What happened?"
Ivan stepped forward like a massive brick wall, planting his broad shoulders directly between Luca and the hospital bed, blocking his path entirely.
"Stop right there, Luca," Ivan said, his voice dropping into a lethal, glacial whisper that echoed off the sterile walls.
Luca stopped dead in his tracks, his fake tears instantly drying up as he locked eyes with his lifelong best friend. The cold, calculating glint of a cornered predator flashed momentarily across Luca’s face before he quickly tried to mask it with righteous indignation.
"What are you talking about, Ivan? Get out of my way! That is my wife in that bed!" Luca shouted, pitching his voice so the nearby nurses would hear. "Have you lost your mind? You're keeping a husband away from his dying wife?"
"She isn't dying anymore, Luca," Ivan said, stepping closer until they were chest-to-chest. "Thanks to the fact that she rolled into my trauma bay instead of a morgue. And thanks to the fact that I pulled your custom-ordered, restricted Dexopraline patch right off her collarbone with my own two hands."
Luca’s face went completely white. Every drop of blood seemed to drain from his skin in a fraction of a second. The arrogant posture crumbled, leaving behind the shivering frame of a desperate man who realized the trap had just snapped shut around his neck.
"I... I don't know what you're talking about," Luca stammered, taking an involuntary step backward. "You're delirious. You've been working too many hours—"
"The police are already at your primary pharmacy," Ivan interrupted, his voice ringing with absolute finality. "They have your digital audit logs. They have the compounding registry showing you pulled ten micro-patches under a fake off-site trial code. And right now, Detective Rachel Vance is interviewing your mistress, Sarah Moore, who is currently spilling every single detail about your little downtown love nest and your plan to inherit Jessi’s multi-million dollar estate."
Jessi stared up from the hospital bed, looking at the man she had loved, trusted, and shared a bed with for twenty-five years. All the illusions, all the decades of shared history, dissolved into dust in a single heartbeat.
"You bastard..." Jessi whispered, her voice trembling with a rage hotter than any fever. "You planned to kill me in my sleep just so you could run off with a twenty-five-year-old assistant and cash out my family's shipping shares."
Luca looked back and forth between his broken wife and his enraged best friend of forty years. He realized there was no escape. The mask was gone.

Without uttering another word, Luca turned sharply on his heel, lunging toward the door to make a desperate run for it.
He didn't make it two steps.
